Lesson Application Information
In order to better serve my future students, I've prepared an on-line form
in which they can express to me their goals and expectations regarding
piano lessons and study. This form is also a student's application
into my piano studio. A parent or guardian must submit the form
for any child under the age of 18. Currently, I am only accepting children ages 5 to
12 in my lesson spots. To recieve, by mail, a hardcopy of
the information contained in this website and lesson application forms with
